This form is for existing patients who need to refill a prescription from our dispensary. Once you submit this form our administrative staff will review your order and get it ready for you to pick up. They will give you a call to let you know it is ready, take payment and arrange a time for it to be picked up. As our products are professional formulations, some may have limited refills and require a follow up visit to discuss the next step in your program and if you need to continue on the product or not.
